CANADA PENSION PLAN

Canada Pension Plan (CPP) provides death benefit for those who have paid into the plan during their lifetime. There may also be a survivor’s benefit for spouses of those who paid into the plan who are not receiving their own benefit. You can get the forms you need to apply for these benefits from the staff at Chapel Ridge 1-800-277-9914 or by downloading them online at www.servicecanada.gc.ca

DEATH CERTIFICATES

Chapel Ridge provides as many copies of the Funeral Director's Proof of Death Certificate as you need for lawyers, financial institutions and others in order to help settle the estate. If you need an official Medical Certificate of Death from the Province of Ontario , you can apply for it through the Office of the Ontario Registrar General.
